How it works
Lifts the estrogen brake
Estrogen tells the brain to slow testosterone production. Enclomiphene blocks that signal at the hypothalamus and pituitary.
More LH and FSH
With the brake lifted, the pituitary releases more LH and FSH, the hormones that drive the testicles.
Your own testosterone, not replacement
The testicles make more testosterone themselves, so there is no shutdown of natural production.
Fertility preserved
Because FSH rises too, sperm production continues, unlike testosterone therapy, which suppresses it.
Enclomiphene is not FDA-approved; it failed to win approval despite phase 3 trials and is available only through compounding pharmacies. Clomiphene, its parent drug, is FDA-approved for women and used off-label in men.

A note from Dr. Wright
For a man with low testosterone who still wants children, or who would rather not start testosterone for life, I like to try restoring his own production first. Enclomiphene does that with a daily pill, and unlike testosterone it doesn’t shut down fertility.

Men with low or low-normal testosterone who are not on testosterone therapy, especially those who want to keep their fertility.
Who shouldn’t take it?
Men already on testosterone (hCG is the better fit), and men with liver disease, a history of blood clots, or vision problems on a similar drug before.
Why labs before starting?
Low testosterone with a low pituitary signal can come from a pituitary problem. We check prolactin and related labs first so nothing needing its own treatment is missed.
Enclomiphene or clomiphene?
Enclomiphene is the active half of clomiphene without the part that lingers and acts like estrogen, so it tends to cause fewer side effects. Clomiphene costs less and is FDA-approved.
I’m already on testosterone. Is this for me?
No. Enclomiphene can’t work while testosterone therapy is shutting down the pituitary signal. See hCG instead.
Is enclomiphene legal?
Yes, when compounded for you by a licensed 503A pharmacy. It is not FDA-approved, and the FDA is still evaluating whether it can be compounded long-term.
