The science

How it works

Mimics ghrelin
It activates the ghrelin receptor, the same switch your hunger hormone uses to signal the pituitary to release growth hormone.
Amplifies natural pulses
It makes your body’s own growth hormone pulses bigger without changing their rhythm, raising levels toward those of younger adults.
Works all day from one pill
A single daily capsule keeps IGF-1 elevated for about 24 hours.
Deeper sleep
In a sleep study, it improved sleep quality, including more REM sleep.

Unlike most peptides here, ibutamoren has randomized human trials, including a 2-year study. It is not FDA-approved, and its original developer stopped pursuing approval.

Side effects

Side effects and what to expect

Increased appetite Common · First weeks
Expected, since it works through the hunger hormone receptor. Evening dosing helps.
Water retention or swelling Common · First weeks
Usually mild. Tell us if your ankles or hands swell.
Higher blood sugar Common · Ongoing
Seen in trials. We check fasting glucose and A1c.
Next-day grogginess Uncommon · If taken late at night
Take it earlier in the evening.
Joint or muscle aches Uncommon · First month
Often eases. The dose may come down.
Numbness or tingling in the hands Uncommon · Any time
Let us know. It can be a sign of fluid retention.

A note from Dr. Wright

Ibutamoren is the one growth hormone option that doesn’t need a needle, and it has real human trials behind it. Those same trials showed it can raise blood sugar and cause fluid retention, so I screen carefully first and check labs along the way.

Questions

Common questions

Who shouldn’t take it?
People with diabetes or prediabetes, active or past cancer, heart failure, or who are pregnant or breastfeeding. It is also banned in tested sports. We screen for these before prescribing.
What did the human trials show?
In a 2-year trial in adults 60 to 81, it raised growth hormone and IGF-1 to young-adult levels and increased lean mass, but it did not improve strength or function, and it raised fasting blood sugar. A separate trial in elderly hip fracture patients was stopped early after a small number developed heart failure.
Why do I need labs?
The main risks, higher blood sugar and fluid retention, show up on labs and in symptoms before they become problems. We check IGF-1, fasting glucose, and A1c.
Is it a peptide?
No. It is a small molecule that acts on a peptide hormone’s receptor, which is why it works as a pill.
Ibutamoren or an injection like sermorelin?
Ibutamoren is easier and has more human data. Sermorelin works through a different receptor and doesn’t drive appetite. Dr. Wright will help you choose.
